A. HOW TO FILL UP THE APPLICATION FORM
Go to the website "http://www.viswass.org/"

In the home page, you will find the following link. Click on the link and the form will appear onscreen within a few seconds.Online Application Form
  1. Application Number will be auto-generated when you submit the application form.
  2. Enter the Bank Draft number.
  3. Enter the name of the designated Bank(with Branch Name).
  4. Enter your Full Name as in 10th class certificate.
  5. Select Course of your selection corresponding to MFC / MSW.
  6. Enter the name of your Father as in 10th class certificate.
  7. Enter the name of your Mother as in 10th class certificate.
  8. Enter your Date Of Birth as specified in your 10th class certificate.
  9. Select your Gender (male/female).
  10. Enter the name of your Native State as specified in your "Permanent Residence Certificate".
  11. Select your First preference for the exam center.
  12. Select your Second preference for the exam center.
  13. Academic Details : Here, you are asked to enter all relevant information such as Educational Qualification, Year of Passing and name of the Board/University as applicable to you.
  14. Enter your Communication Address.
  15. Enter the phone number of any fixed landline or WLL (if any)
  16. Enter your mobile number (if any) to receive any important information through SMS.
  17. This is your testimonial and you must agree to abide by the terms and conditions declared here in by putting a check mark for "I accept the Declaration". When you accept declaration, a Submit button will appear just below. Click on the Submit button to submit the application form.
After proper submission of the Application form, your Application number will be generated and an Acknowledgement Card will appear immediately. Please take a print-out copy of this Acknowledgement card.
IMPORTANT
If you have printing facility, please make a print out of this Acknowledgement Card. If you don't have printing facility, then please note down your Application number.
After receipt of Bank Draft, you will be allotted a Roll number and you will be allowed to download your Admit card.
While downloading your Admit card online, you will be asked to enter your Application number and Date of Birth for confirmation of your authenticity. Hence, while filling up the application form, please supply your correct date of birth.
Please remember that last date of submission is the 15 days from the publication of results, while last date of receiving bank draft is 25-01-2010.
B. HOW TO SUBMIT BANK DRAFT
Make a Bank Draft of Rs. 250 (Rupees Two Hundred Fifty only) for MSW & MFC and Rs. 200 (Rupees Two Hundred only) for BSW & BBA in any Nationalized Bank of India in favour of
"VISWASS, PAYABLE AT BHUBANESWAR"
While filling up the Application form online, enter the Bank Draft number in field no.2 and name of the designated bank (with branch) in field no.3 of the application form.
You will get the Application number after your application is successfully submitted online.
Please write your Application number and put your Full signature (to be signed by the applicant) on the backside of the bank draft.

NOTE :
  • Bank draft without application number and full signature shall not be accepted.
  • Application form shall not be considered until your draft is received.
  • If you want to apply for two different courses, then you have to submit two different application forms with two separate bank drafts (for Rs 500/- each)
  • After receipt of the Bank Draft, you will be allowed to download the Admit Card containing the Roll number and Venue of the Entrance test.
C. HOW TO GET ADMIT CARD
When you are going to download the Admit card, you must be sure that you have successfully submitted the Application form online and sent Bank draft to VISWASS, payable at Bhubaneswar.
When you click to download the Admit card, you will be asked to enter the following information for verification of your authenticity -

(1) Application number - It is given to you when you have submitted your application form
(2) Date of Birth - It is asked to enter for confirmation of your identity
NOTE :
  • When your draft is received, Admit Card will be issued online and you can download your admit card.
  • Without Admit card, you shall not be allowed to appear for the examination.
  • However, for any queries, you can contact us through the "CONTACT US " page given in the website.
  • When you get your Admit card, your new Roll number along with the Application number may be referred to for any future correspondence.
  • Please quote Roll number and Application number in all your future correspondence.
